Tell us a little bit about your role and a day in the life.
I coordinate care for patients with chronic medical conditions. I begin with an assessment of my patients’ medical needs and services and then develop an integrated care plan. Daily, I work alongside my patient’s provider, therapist and caregivers to help educate and manage all aspects of their healthcare.
